Georgia law requires children from kindergarten to grade 12 to be properly immunized and vaccinated. All students must have Form 3231 marked "Complete for School" and on file. You can only obtain this form from a physician in Georgia.
Medical and religious exemptions from vaccinations are permitted in certain circumstances.
All children must have the following immunizations, administered age-appropriately:
- Pertussis (under age 7)
- Hepatitis B
- Hib disease (under age 5)
In addition, children who are entering 6th grade are required to show the following:
- proof of protection against measles (two doses given on or after the first birthday and at least four weeks apart or a positive serology)
- proof of protection against varicella or chickenpox (vaccination dates, positive serology, or a health care provider’s interpretation of a parent’s description of disease history).
