How to Get a Replacement Birth Certificate in Georgia

It's time to sign up for school or sports, and you can't find your child's birth certificate. Fortunately, Georgia makes it easy for you to obtain a replacement copies of birth certificates for children born in the state. You can even get one on the same day in most cases if you go in person. 

1. By mail: 
To request a birth certificate by mail, you must provide a signed copy of the required information, a copy of your photo ID (your child's ID is not required), and payment. 

Required information
You can print the online form and fill it out by hand, or you can fill out the form online and print it. If you don't have a printer, you can write out the required information.

    • Full name of person shown on the birth certificate (last name at birth if female)
    • Date of Birth (month, day, year)
    • Place of Birth (city, county),
    • Current age
    • Sex
    • Race (optional)
    • Full name of mother (include mother's maiden last name)
    • Full name of father
    • Relationship to the person named on the birth certificate requested
    • The number of certificates requested

    Fees and Payment

    The cost is $25 for one birth certificate. You can purchase multiple copies at the same time for an additional $5.

    Payment must be made by certified check or money order made out to Vital Records Services. Personal checks and credit or debit cards are not accepted. Do not mail cash.

    2. In person:

    If you don't have time to request a birth certificate via mail, you can go in person to the state Vital Records Office and get the certificate on the same day. You'll have to wait in line, but the process is fairly quick if you have the correct information and payment. You must fill out a form with the required information at the Vital Records Office. You'll also need to bring a copy of your photo ID (your child's ID is not required), and payment. 

    You may be able to go to the Vital Records Office in the county where your child was born to receive a birth certificate. Check with the county office before you go to make sure you can get it on the same day.
